Submission form
When submitting material for diagnostic examination, a carefully completed submission form must be enclosed. The form must contain the date of the sampling of the test material, information on the sort of the submitted material and the reason for the submission, including information on the course of disease, treatment and autopsy findings. In some cases, information on the feeding of the animals can also be of importance.
We recommend that the submission form is placed into a plastic pocket, so that it will not be in direct contact with the test material.
